2017 House League Coaching Staff Application Form
House League Team Staff Application Information: Team Staff are designated as Coach, Trainer, Assistant Coach, Manager and Assistant Trainer. Alliance Hockey recommends that all coaching staff have NCCP Certification. The same person cannot be registered on a team’s Alliance Roster as both a coach and trainer. Head Coach – Applicant must have completed a Hockey Canada NCCP certification - Coach Stream or higher and the RIS/Speak Out module (except at the levels of Novice and below where the HCIP may be the minimum NCCP certification requirement) Trainer - To be eligible to be a Trainer or Assistant Trainer the applicant must have successfully completed a HDCO Hockey Trainers Certification Program (H.T.C.P.) at Level 1 or higher and the Speak Out Module. Assistant Coach – Minimum Certification – Speak-out Module (Respect in Sport - Coach module); recommended Coach Stream certification Manager – If on the bench minimum requirement is Speak-out Module (Respect in Sport - Coach module) Scheduled dates and times of required clinics will be posted as the 2017 - 2018 season approaches.

Coach/Trainer Experience

I submit all of the above information as accurate and a true reflection of my intent.   I understand that as per Alliance Hockey Harassment, Abuse & Misconduct Policies & Procedures, July 2014, a Criminal Record Check (CRC)/Vunerable Sector Screening (VSS) is required every three years to be eligible to be a Coach, Asst Coach, Manager, Trainer or on-ice helper (both Rep & House League) with a team in Sarnia Hockey.  If I am successful in my application I understand I must contact Wayne Scarrow, Past President of the Sarnia Hockey Association, to either confirm that I have had a Criminal Record Check (CRC)/Vunerable Sector Screening (VSS) done in the last three years, or arrange to have one done (these are prepaid by Sarnia Hockey).  A Criminal Offence Declaration Form (CODF) must be submitted in alternate years.

All Coach, Asst Coach, Manager, Trainer or on-ice helper (both Rep & House League) with a team in Sarnia Hockey must submit a valid Criminal Record Check (CRC)/Vunerable Sector Screening (VSS) or CODF prior to December 31st of the current season or they will be suspended from their role and not be allowed in the dressing room or on the bench.
