Baseline Testing (Concussion Management)
Please find below a link and instructions for our team’s baseline concussion test.
All representative players must take this test prior to their first league game.
No player will be allowed on the ice until they have completed the test.

Note:
Please ensure that the test is taken in a quiet room with no interruptions.
The test should be treated like an exam.
Please remove yourself from any distractions that might affect your concentration.
This year’s program has a built in response device that picks up on the subtitles of response. If a player conducting the test is not responding at a 100%, the test will come up invalid and he will have to re-take the test under supervision.
MAKE SURE YOUR POP UP BLOCKER IS OFF OR TEST WILL NOT LAUNCH.
Step 1: www.impacttestonline.com/CMRG
Step 2: Under select Province ONTARIO.
Under organization select Sarnia Minor Hockey and code.
Step 3: Access Code 76B1811CD0 (the last number of the code is zero)
Step 4: Select your team under organization and begin test.
Please set aside 25 minutes for completion. It’s best to print this page to begin testing.
Players are asked to complete this test prior to the practice on Saturday, September 28. Please print the confirmation page upon completion of the baseline testing and bring it to Saturday's practice.
