Hi All,

Please see below regarding the baseline concussion testing that needs to be completed ASAP.



Online ImPACT Concussion Testing Instructions

These tests are a very important aspect of your full baseline testing evaluation. Please complete this test as soon as you get home with no distractions. This test measures both your speed as well as your accuracy for answering various questions. This test is NOT an IQ test and does not measure your level of intelligence but rather your basic processing skills. Try your absolute best!

To access the online baseline testing please follow the instructions below: You will need:
• A computer with a mouse (laptop track pads are not permitted)
• A quiet room free from any distractions (Very Important!)
• The access code for the clinic in which the testing was conducted (see below)

The online test requires your full attention for approximately 30 minutes. Please ensure that you will not be distracted or interrupted during the testing. This test must be taken alone without any help! This is to assess YOU. Avoid taking the test if you are tired or stressed.

If you are having problems accessing the test, please make sure that your pop-up blocker is turned off.

Once you are settled in a quiet room:
1.     Go to: www.impacttestonline.ca/testing/
2.     Customer ID Code: IKFSJL30JL    *This code is not case sensitive*
3.     Select your organization: “Sarnia Hockey Association”
4.     Follow the online instructions until the test is over.

Please perform to the best of your abilities and focus hard! Your testing clinic will receive your test results, and if there is any re-testing required, you will be notified.
