We've added a COVID19 Forms and Tools to our webpage "MENU"
Please use our webpage "MENU" to access the link to the Ontario COVID19 Self Assessment Tool. Other items on this page are required for team officials for contact tracing records. This menu can be directly seen on a desktop computer or can be selected on the websites mobile version.
Self Assessments are required for all coaches, players, staff, and spectators before entering the facility and must be completed within 2 hours of entering the facility. Team staff will not be accepting paper copies of self assessments. We are doing our best to make record keeping simple and easy to access. As well this is a very good "NO TOUCH" method and is preventing possible COVID 19 community transmission
