If you have a player on one of our MD, B/BB or A/AA teams, please take the time to complete a quick evaluation form on the Head Coach and/or Assistant Coach(es) for your team.
Parent/Player information will not be shared with the coaches. Only the Website Director and the Vice-President Rep Teams will see this info, in order to prevent duplicate results, quality control and for potential feedback.
