2020-2021 Sarnia Hockey Apparel Form (Sarnia Hockey)

I understand that by submitting this order form I am required to pay for all goods at time of delivery.  I have re-read my order and ensured that all information is correct.  I understand that failure to pay will result in my membership status with Sarnia Hockey and it's governing bodies be "NOT IN GOOD STANDING" and may result in my player(s) being suspended from any Hockey Canada sanctioned organizations and events until full payment is recieved
