I submit all of the above information as accurate and a true reflection of my intent. I understand that a criminal record check is required to be eligible to coach, be a trainer or on the bench with a team in Sarnia Hockey. If I am successful in my application I understand I must contact the Past President of the Sarnia Hockey Association, to either confirm that I have had a criminal record check done in the past, and that it is still valid, or arrange to have one done (these are prepaid by Sarnia Hockey).
All coaches, managers and trainers must submit a valid police check prior to December 31st of the current season or they will be suspended from their role and not be allowed in the dressing room or on the bench.
