I affirm all of the above information as accurate and a true reflection of my intent.   I understand that a criminal record check is required and must be submitted to the OHF to be eligible to participate with a team that is registered with Sarnia Hockey.  If I am successful in my application I understand it is my responsibility to complete the entire VSC Submission and Validation process as prescribed by the OHF

I understand that it will be my responsibility to provide payment for my Vulnerable Sector Check and apply for re-imbursement from SHA

I understand that the OHF is requiring all coaches, managers and trainers in the HL and HL Select Categories to submit a valid police check within 3 weeks of registering for the current season or they will be permitted to participate with SHA including in the team dressing room or on the bench.

I authorize Sarnia Hockey Association Representatives (Members of the Board of Directors or Coach Selection Committee) to:

1. Collect personal information appropriate to the position I am applying for
2. Contact any of my listed references
3. Validate and Verify any information provided on this application for the purpose of vetting and screening HL Select Coach Applicants
