Please complete this form. Once received and approved by SHA, an NRP Passport will be issued for the player listed and emailed to the email address given. A copy will also be sent to the OMHA Association listed on the form, OMHA Head Office, Alliance Head Office and SHA Registrar for record keeping. Please remember that players are limited to ONE NRP Passport per season. Players requesting an NRP Passport MUST attend SHA A/AA and BB tryouts and be released prior to being eligible for an NRP Passport.
