All volunteers with Sarnia Hockey Association are required to complete a Vulnerable Sector Criminal Reference Check (VSC) that will be considered valid for 3 Years from date of issue and expires 3 years from the date of your VSC approval date.  In addition to a VSC, Sarnia Hockey requires an annual submission of a CRIMINAL OFFENCE DECLARATION FORM (CODF). All VSC & CODF are to be submitted to our Chairperson & Past Chairperson for input into the Hockey Canada Registry.  They are the only persons that will see your VSC status for those with privacy concerns.  Once reviewed the SHA Chairperson will update your Hockey Canada Profile that a VSC or CODF has been completed and your status is current.  If there are any questions the SHA Chair or Past Chair will contact you

Volunteers are required to apply to their respective Police Service Provider. If requested by the Police Service Provider, a volunteer confirmation letter can be provided by emailing the SHA Chairman and requesting a volunteer specific letter.

If you live within the "City of Sarnia" please apply with the Sarnia Police Service (Online Application Link)
If you live outside Sarnia, but within Lambton County please apply at your closest OPP Branch
If you live outside Lambton County(Strathroy, London, etc) you must see your local Police Service
