Concussion Testing Printout
Please complete this online test, must complete test before first league game.  No one allowed on ice until completed!

Baseline Testing (Concussion Management) Instructions 

 

Note:

 

Please ensure that the test is taken in a quiet room with no interruptions.  The test should be treated like an exam.  Please remove yourself from any distractions that might effect your concentration.  This years program has a built in response device that picks up on the subtitles of response.  If a player conducting the test is not responding at a 100% the test will come up invalid and you will have to re take the test under supervision. 


MAKE SURE YOUR POP UP BLOCKER IS OFF OR TEST WILL NOT LAUNCH


Step 1:  www.impacttestonline.com/CMRG 

 

Step 2:  Under select Province ONTARIO.

 

Under organization select Sarnia Minor Hockey and code.

 

Step 3:  Access Code        76B1811CD0   the last number of the code is zero

 

Step 4:  Select your team under organization and begin test.  


Please set aside 25min for completion.  It’s best to print this page to begin testing.
