Registration Policy:
This form should be submitted only after funding has been approved and the requestor holds a certificate, approval or other confirmation funding will be provided.
Sarnia Hockey will not provide a special First Shift registration link unless:
1) Proof of Funding is attached
2) Full Payment has been submitted and validated by the SHA Treasurer
First Shift Refund Policy:
Sarnia Hockey will not provide refunds for First Shift registration unless:
1) Full payment was provided by the participant, their parent or guardian; and
2) Refund Request/Withdraw Request was received by Sarnia Hockey Association Administrators before Nov 1 2024
