2021/2022 - U12A/AA TRYOUT DATA CAPTURE, U12 A/AA, 2021-2022, AA (Sarnia Hockey)

I understand that this information is being collected in order to facilitate the administration of COVID 19 screening requirements mandated by the Government of Ontario, City of Sarnia, The OHF, Alliance Hockey & Sarnia Hockey Association

1) I have reviewed the above information and it is correct. 
2) I understand that 48hrs prior to Tryout #1 I will receive an email requesting me to download the SPORTSHEADZ MOBILE APP to register my player and at least one guardian
3) I understand that only registered guardians are able to enter the arena with the players for tryouts
4) I understand that the City of Sarnia mandates 1 guardian per skater may enter the arena during this tryout session.  No additional spectators are permitted
5) I understand that as a guardian I will declare my players attendance to tryouts and complete the health check form for them and myself on the SPORTSHEADZ Mobile App prior to arriving for tryouts
6) I understand that if I do not receive the SPORTSHEADS MOBILE APP INVITATION by 8AM - 16 Sept 2021 I will need to contact the U12A/AA Manager CLICK HERE TO EMAIL
7) I understand that it is a requirement of ALLIANCE HOCKEY and SARNIA HOCKEY that all participants over the age of 12 including players, coaches, officials and volunteers are required to have their first Health Canada Approved COVID 19 VACCINE by 17 Sept 2021 and their second Health Canada Approved COVID 19 VACCINE by 31 Oct 2021.  All exemptions must be submitted to SHA Chair prior to 17 Sept 2021

I understand that inadequate or incomplete submissions of attendance and a health check via the SPORTSHEADZ Mobile App could cause my player to be denied access to the arena until adequate COVID19 screening can take place by a registered team official
