Coaches and Parents - Please fill out this form as accurately as possible. This form was developed to make early identification of potential hold ups while confirming and approving team rosters. Teams that do not have an approved roster will be denied the privilege of participating in any events outside of the SHA Novice HL Schedule. Failure to follow the registration and roster procedures for SHA and Alliance Hockey will result in the suspension of ice privileges.
